A 52-year-old Spring Valley resident was killed at KJ Tile Warehouse in West Haverstraw following a forklift accident Thursday morning.
Haverstraw first responders were called to the scene at 8:40 a.m. and found Carlos Ramirez-Hernandez lying on the floor surrounded by broken boxes of tile. He was pronounced dead at the scene, police said.
Police said while operating a forklift, Ramirez-Hernandez reversed into commercial metal shelving containing pallets of tile. The impact of the forklift striking the shelving caused boxes of tile to fall from the shelving and struck Ramirez-Hernandez.
This incident is being investigated by the Town of Haverstraw Detective Division, Rockland County Sheriff’s Department, Town of Haverstraw Building Department, and OSHA. According to reports, police have ruled out foul play.
